What makes a PC officially a member of a secret society? E.g., what do you as a player need to do to officially join?

What if a PC finds that the requests of his/her secret society don't match up with their sensibilities? E.g., the society asks the PC to do things that he/she finds reprehensible. What are the consequences if the PC doesn't do as they ask?

How does a PC leave a secret society? What are the consequences?

You say "I'm a member of this Secret Society." and that's about it.

Some adventure may give you -1 Fame with your Secret Society. Some adventures, it might not matter. You can of course attribute your own consequences (leave your society). You might also not be able to take special Paths related to your society if your Fame is beneath the requirement.

I would say it depends on the Society. Some characters can't leave (Laerestri maybe?) while others are probably fine to go "I've left the Society" (Feathermen).

Anyone can be in a Secret Society; the "paths" exist to show that you're more tightly integrated into the society.

Some societies have official invitations, but all of them you can just say "I'm a member."

Generally, if you've joined a society, it's because you agree with their principles. If they're asking you to do something... odd, there may be something else going on. Sometimes the consequences are loss of fame within the organization. Sometimes you just don't get bonus experience. Sometimes, if you're actively taking a stand, there will be other consequences.

Leaving a society is a little tricky. If you've just said you're a member but you've never actually completed a mission for them, then you probably don't know much you can reveal to others. They're not going to be overly concerned. However, if you've become an integral part of the society and defect, they are not going to be pleased. At the very least you'll be a pariah in their eyes. More likely, they'll try to hunt you down and hurt or kill you, and make things difficult for you in the future. Currently there are no official mechanics for this but we're working on developing them. I believe as it currently stands, you receive double your existing Fame in Infamy with them. If that puts you at 5 Infamy or greater, they will actively work against you. 10 Infamy or greater will give you the Flaw of Marked Man, and their agents will try and kill you (if they can) or manipulate situations to have you killed.
